Understanding IPF  


Pulmonary fibrosis refers to scarring of the lung tissue (fibrosis of the lungs). Idiopathic pulmonary fibrosis (IPF) is defined as a specific form of chronic, progressive fibrosing interstitial pneumonia of unknown cause, primarily occurring in older adults, limited to the lungs, the most common form of idiopathic interstitial pneumonia, causes progressive pulmonary fibrosis. is a type of chronic lung disease characterized by a progressive and decline in lung function.



WHAT CAUSES PULMONARY FIBROSIS?


The cause of IPF is unknown but certain environmental factors and exposures
have been shown to increase the risk of getting IPF. Cigarette smoking is the best recognized and most accepted risk factor for IPF, and increases the risk of IPF by about twofold .Other environmental and occupation exposures such as exposure to metal dust, wood dust, coal dust, silica, stone dust, biologic dusts coming from hay dust or mold spores or other agricultural products, and occupations related to farming/livestock have also been shown to increase the risk for IPF



 *   Chronic inflammatory processes (sarcoidosis , Wegener's   granulomatosis)

 *   Infections

 *   Environmental agents (asbestos, silica, exposure to certain gases)

 *   Exposure to ionizing radiation (such as radiation therapy to treat tumors of the     chest)

 *   Chronic conditions (lupus, rheumatoid arthritis)

 *   Certain medications

In some people, chronic pulmonary inflammation and fibrosis develop without an identifiable cause. Most of these people have a condition called idiopathic pulmonary fibrosis (IPF) that does not respond to medical therapy, while some of the other types of fibrosis, such as nonspecific interstitial pneumonitis (NSIP), may respond to immune suppressive therapy.

Synonyms (other names) for various types of pulmonary fibrosis that have been used in the past include chronic interstitial pneumonitis, Hamman-Rich syndrome, and diffuse fibrosing  alveolitis.



SIGNS AND SYMPTOMS OF IPF


 Ordinarily, every patient are unique in their own different ways, as a result of this, the signs and symptoms they experience varies. Few but not limited to the under listed are the most common signs of IPF.  



   * Cough (Dry, non-productive cough on exertion)

   *  Shortness of breath or difficulty breathing

   *  Decreased exercise tolerance

   *  Clubbing of the digits, a disfigurement of the finger tips or toes

   * Abnormal pulmonary function test results, with evidence of restriction and impaired gas exchange.

Symptoms and signs of idiopathic pulmonary fibrosis typically develop over 6 month to several years. 



WHAT ARE THE STAGES OF IDIOPATHIC PULMONARY FIBROSIS?




Idiopathic pulmonary fibrosis has been loosely staged and categorized as mild, moderate, or severe, or early versus advanced. The staging is usually based on pulmonary function tests, although these stages are considered arbitrary.




There are some newer proposed staging systems:

 *        Mortality risk scoring system: This is based on four predictors

*         Age,         

*         Recent respiratory hospitalization,      

*         Baseline forced vital capacity (FVC, a measure of lung function), and

*         24-week change in FVC.  



HOW IS IPF DIAGNOSED?



The diagnosis is usually made by a specialist, and so for most patients this means being referred by their general practitioner to the hospital. Most people will need to have the following investigations.

·   A chest x-ray.

This may show signs of the scarring even early on, so some people will be diagnosed by an abnormal chest x-ray before they develop symptoms.

·   Lung function tests.

These are breathing tests to show how well your lungs are working. They are also used later on to monitor the severity of the disease and how it is progressing.

· Blood tests.

These are done to find other causes of lung disease.

·  CT scan.

This is a special x-ray that gives a more detailed picture of your lungs. There are characteristic features on the CT scan that can allow your doctor to make the diagnosis of IPF in about half of those with IPF, others requiring a biopsy if the changes on the CT are not classical.

· A bronchoscope

  This involves passing a telescope down into the lungs to collect samples.

· A biopsy

If the diagnosis is not clear after the above tests some patients may need to have an operation with a general anesthetic to remove one or more pieces of lung for more detailed tests. This procedure is called surgical lung biopsy and sometimes is the only way to make a firm diagnosis.


   What will happen if my condition starts to deteriorate?


Unfortunately most patients with IPF will become more breathless with time and some will die of the disease. Sometimes the disease does not progress; more usually there is continuing scarring with increased breathlessness.

Some patients have acute exacerbations when breathlessness increases suddenly. This is often confused with pneumonia and needs urgent hospital care with a repeat CT scan and different treatment.

WHO CAN DEVELOP IPF?



Most people with IPF develop their symptoms between the ages of 60 to 85 years and the disease is uncommon below the age of 50 years. Men are affected more commonly than women. Idiopathic means of no known cause, however the disease is more common in smokers, people who have worked with metals, wood, stonecutting, and working with animals or birds. A similar disease may complicate rheumatic diseases such as rheumatoid arthritis and scleroderma, and may be due to treatments for these diseases and some other drugs. IPF is not ‘passed on’ to children, but sometimes occurs in several members of a family suggesting that genes may be involved in causing the disease. A search for underlying causes is an important part of the initial investigation.

Azathioprines suppress the body's immune system in order to keep it from damaging its own organs. In the case of pulmonary fibrosis, the medication could help protect the lungs from immune system attack and further scarring. However, while it does help some patients, the effectiveness of this medication for patients with pulmonary fibrosis has not yet been confirmed. Common side effect of using Azathioprine include:  signs of infection (fever, chills, sore throat, body aches, weakness, muscle pain, flu symptoms); severe nausea, vomiting, or diarrhea, pain or burning with urination.



Mycophenolate Mofetil: This drug is another type of immunosuppressant that doctors commonly prescribe to patients with pulmonary fibrosis. They are sometimes used in combination with corticosteroids. Common side effect include: nausea, vomiting, stomach pain, diarrhea, or constipation, headache, mild weakness, swelling in your hands or feet, numbness or tingly feeling.



Nintedanib: This anti-fibrotic medication is another drug often used to treat idiopathic pulmonary fibrosis. When the disease is still in its early stages, this drug can be effective at slowing disease progression and lung function decline. Common side effect include:  sudden vision problems, slurred speech), severe stomach/abdominal pain, abdominal swelling.



Corticosteroids: Corticosteroid medications like Prednisone help treat pulmonary fibrosis by suppressing the immune system. This helps to reduce inflammation in the lungs but requires careful monitoring by a doctor since it can have many serious side effects. Common side effect include:   weight gain, increased vulnerability to infection, cataracts and glaucoma (eye disorders), thinning of the skin, bruising easily, osteoporosis (fragile bones), hypertension (high blood pressure), diabetes.



Proton Pump Inhibitors: Proton pump inhibitors are often used to treat gastrointestinal diseases like peptic ulcers and GERD, but they can also treat types of pulmonary fibrosis. These medications don't target the lungs or immune system, but instead work by blocking excess stomach acid production.

In some cases of pulmonary fibrosis, excess stomach acid leaks into the lungs, causing irritation and scarring. By reducing the amount of acid your stomach produces and quelling acid reflux, proton pump inhibitors help prevent the lungs from getting further inflamed. Common side effect include: headache, nausea, diarrhea, abdominal pain, fatigue, and dizziness. Infrequent adverse effects include rash, itch, flatulence, constipation, anxiety, and depression. Also infrequently, PPI use may be associated with occurrence of myopathies, including the serious reaction rhabdomyolysis.

Ich habe seit 3 Jahren eine idiopathische Lungenfibrose, ich habe verschiedene Medikamente ausprobiert und bin sehr an Gehübungen beteiligt. I was diagnosed with IPF 3 years ago and  I am on medication and have been through numerous chest infections fatigue depression the whole works really but of late can’t seem to shake infections off and coughing has fractured my ribs and burst blood vessels in my eye. Having IPF can be lonely, isolating and scary. I have to avoid walking up hills or doing strenuous exercise, so I have to plan where I’m going, where I’m going to park, especially if I have to do a lot of talking in meetings etc. But when people share their stories of coping with IPF or substance use disorder, it can provide inspiration and hope and be a welcome reminder that you are not alone in your challenges. I had suffered from IPF as long as 2 and half years, I had no idea what was happening and didn’t know where to turn for help. It was hard then because I didn’t understand the symptoms earlier. When you struggle with a disease, many won’t believe you. Early detection of PF and early treatment could have prevent full-blown IPF and improve the long-term prognosis. There was a time I decided to get away from everyone, moving into a serene house in the woods with a pond and vocal wildlife. I said, “I loved to gaze at the moon and listen to an orchestra of fireflies and bullfrogs”.  As do most families, I went through many sleepless nights and periods of intense grief. I cannot express the pain and anguish that came over me.  We (my wife and I) fought for proper medical care and humane treatment. We did everything we could and we did it with love, with intelligence, with experience that threatens my life. We have ALL given pieces of ourselves, sacrificed our own way of life for this disease and have spent many late nights worrying and praying. Behind the closed doors of our homes or a therapist’s office or a specialist office we put our heads together to decide what is best for me and sometimes most of our choices work but more often we are less than successful. There was very little support from friends, family and the community. I racked my brain and kept thinking of all the things I could have done differently to prevent such a horrible illness.

It was such a relief when someone told me about the Pulmonary Fibrosis Foundation of Australia. There I found other people going through very similar situations as me.
